Was your identity stolen and used to set up a new account without your authorization?

If so, please click Begin Claims Process to initiate your theft of identity claim. If you have already filed a claim with an outside collection agency, you do not need to file another claim here.

Be advised that our investigation will require you to subsequently submit the following documentation:

  1. Completed and signed Identity Theft Victims’ Complaint and Affidavit (download here)
  2. Proof of Identity (current State/Government issued Photo ID; Military ID not accepted)
  3. Proof of Residence (for the period of time when the event occurred)
    1. One of the following (must provide copy of full bill)
      1. Utility bill (cannot be an AT&T bill)
      2. Insurance bill (health, homeowners, renters, or automobile)
      3. Lease, mortgage, or HUD agreement
      1. Completed and signed Identity Theft Victims’ Complaint and Affidavit (download here)
      2. Two forms of ID
        1. Proof of Identity of authorized party (current State/Government issued Photo ID; Military ID not accepted)
        2. One of the following
          1. Valid Business license, registry of non-profit organization, articles of incorporation (first 3 pages)
          2. Certificate of good standing issued by Secretary of State
